Thank you for your interest in our programs.  We are honored you would consider allowing us the chance to support your family in your journey. To start an application process follow these five easy steps.   

• Contact our program staff and provide the child's diagnosis from a school or physician.

• When eligibility is confirmed, we send an email containing the link to the online application forms as. This form requires health, behavioral, and insurance information.

• Meeting – This will take place in our office. Please bring the completed application form as well as the documented proof of diagnosis. We review your application together and formulate any necessary action plans. During this time we provide a detailed explanation of services, and what we need from you in order to make your experience with SHARE the most rewarding.

• Let us care for you! Services start after all information is collected and necessary action plans completed and confirmed. The first time for anything new can be nerve-racking. We encourage you to give each of our programs a chance. We know you will find something or someone who will enhance your time with us, and enrich your journey. Many SHARE parents tell us they have formed supportive and lifelong friendships from those in the SHAMILY (SHARE family).
